This puppy has been examined by a licensed veteranarian and is found to be free from any congenital defects. This puppy has had the dew claws removed, been given the first round of vaccinations, has been de-wormed, and is microchipped.

This puppy has a health guarantee against congenital defects. The purchaser is required to do the following:

1. This puppy must be examined by a licensed veterinarian within 3 business days of purchase. The veterinarian must check for congenital defects along with the puppy’s overall health. A health certificate, or letter, on company letterhead and signed by the veterinarian, must be returned to the breeder within 14 days of purchase. Health guarantee will be considered null and void after 3 business days as the breeder cannot control the puppy’s environment.

2. If, within the first 3 business days, your veterinarian finds a congenital defect, you must contact the breeder within 24 hours to advise me of the problem. A written description, on company letterhead, must also be sent to the breeder. This puppy may be returned, along with the AKC papers, for a full refund or another puppy (if available). All costs, including veterinarian fees are the sole responsibility of the purchaser.

This puppy is covered by a 26 month Hip Guarantee. To be covered:

1. You must have returned the health certificate signed by your veterinarian with in 14 days of purchase.

2. The prefix Mystic Dreams is in this puppy’s registered name, is owned by the original purchaser, is not over 26 months of age, and has not been bred.

3. Hip evaluation has been evaluated by the Orthopedic Foundation for Animals (OFA) and is found to be moderately to severely dysplastic. All costs are the sole responsibility of the purchaser.

If OFA has determined that your puppy/dog is moderately to severely dysplastic, the purchaser has options:

1. You may return this puppy/dog and signed AKC papers transferring ownership back to the breeder and receive another puppy or a refund of your purchase price.

2. You may keep this puppy/dog and the breeder will refund half of your purchase price after the purchaser has returned a letter signed by a veterinarian stating that this puppy/dog has been spayed or neutered The cost of spay/ neuter is the sole responsibility of the purchaser.

The purchaser also agrees that they will not surrender this puppy/dog to an animal shelter or give or sell to an animal research facility or laboratory.

The breeder does not guarantee that this puppy/dog can produce offspring or that the puppy/dog will perform or win in show or agility venues.

The purchaser will not breed this puppy/dog on limited (pet) registration. If the puppy/dog is bred on limited registration, the breeder has the right to file a lawsuit against the purchaser for damages to the breeder's name and reputation and for all costs relating to the lawsuit .

No puppy/dog sold on full registration will be bred until the puppy/dog has had proper OFA and eye clearances.

By signing this Health Guarantee and Agreement, the purchaser is acknowledging that he/she has read, understands, and will comply with the terms of this Health Guarantee and Agreement.
